WHAT IS INCLUDED IN DPC?
•Preventive Care: We provide school, sports and pre-employment physical examinations, annual wellness checkups, order for age specific cancer screening, Pap smears, pelvic and breast exams.
•Acute Illnesses: If you are acutely or suddenly ill, just give us a call and we will see you the same day.
•Chronic Care Management: We provide personalized care plans to manage and control long-standing illnesses with the goal of improving patient outcomes and prevent possible ER visits or hospital admissions.
•Care Coordination: We assist with referrals to specialists if needed and coordination of care with other providers.
•Basic in-office procedures: We perform in office tests if indicated such as EKG, urine dipstick, pregnancy test, random glucose test and AIC, and strep throat swab test. We also perform basic office procedures such as suture removal, wound assessment and dressings, breathing treatments, ear lavage or cerumen (wax) removal, injections and IVs.
Zenith Health & Wellness Direct Primary Care (DPC) gives you direct access to well experienced and culturally competent providers who will give you personalized care planning, listen to your concerns and supply you with health counselling and education.
WHAT IS DIRECT PRIMARY CARE?
•DPC provides direct access to primary care services for in-person or telehealth visits for a monthly subscription fee.
Zenith Health & Wellness offers both direct primary care (DPC) and traditional primary care which relies on health insurance or self-pay to cover the cost of medical services.
WHAT’S COVERED WITH DPC?
•You will have a maximum of 2 visits per week for a total of 8 visits per month. A visit will be either an in-office or a telehealth. Patients may not need to visit 8 times within a month, but the option is there for thorough and continued patient care through follow-ups.
•A patient’s out of pocket costs with or without active health insurance coverage may include prescription medications, laboratory and radiologic tests which are done outside of our office, other specialized testing that would require a referral to a specialist, and any in-house medications or supplies that may be used for treatment.
WHAT ARE THE TERMS?
A minimum of 10 months signed agreement with automatic drafting of payments once enrolled is required. A patient who signs up for 12 months gets the 12th month free. A patient may terminate an agreement 30 days ahead of the next scheduled payment and vice versa.
We offer discounts on couple or family plan enrollment.
